Sierra Summit Children's Academy

• Completed Parent Handbook (pages 19-22)
• Up to date immunization record
• Tuition and registration fee
• Physicians statement for status of child’s health
• Physicians note for medication (including over the counter medication)
• Diapers & wet wipes (if needed)
• Several changes of clothes
• Special weather clothing (coats, hats and gloves)
• Lunch and 2/3 snacks (no nuts or nut products)
• Full-size crib sheet
• Small pillow and blanket
• Any other item that you feel will make your child’s day more comfortable.

A statement of good health will be required within the first 30 days of attendance. This is a statement from the doctor that says the child is okay to attend the school. It can be on a prescription pad or doctor letterhead.
